UK government health forms

Question

When replying to UK government forms from the Department for Work & Pensions, as a footnote can one add this text:
'This letter and all the attachments is intended only for the use of the person ('intended recipient') to whom it is addressed. It contains information that is privileged and confidential. Accordingly any dissemination, distribution, copying or other use of this letter or any of its contents by any person other than the intended recipient is strictly prohibited. If you are not the intended recipient, please contact the sender as soon as possible.'
Or is it best to leave it out.

You are writing a letter.
You can put whatever you want in it.
Whether, in the specific circumstances that you mention, it would have any relevance is anybody's guess.
